How to use your new health plan.

You bought a plan that pays you — fixed cash benefits when things happen, no deductible to meet first, no network you're locked into. This page is everything you need to actually use it. Bookmark it.

Start here — four things this week.

Do these before you need them. They take about fifteen minutes total, and they're what separates "I have insurance" from "I know how to use it." Tap the box as you finish each one.

  1. Register on the ManhattanLife portal

    Your temporary ID card, every policy, claim status, and payments. Choose "I have a life or health policy," enter your name and Social Security number, and it finds your policies automatically. Save the ID card to your phone. Prefer an app? ManhattanLife's Client Services app (Google Play) does the same things — the website is the reliable one.

First few days

ManhattanLife emails your policy documents. Your temporary ID card is in the portal right away.

7–10 days

Printed policies and ID cards arrive by mail. Watch for anything from ManhattanLife by mail or email.

Effective date

Your first premium drafts on or right after your effective date. There's no coverage until that draft clears, so make sure the money's there.

How your plan works.

Your coverage is a package of fixed-benefit policies. Each one pays a set dollar amount when a covered event happens — a doctor visit, a prescription, an ER trip, a night in the hospital — and pays it to you, from the first dollar. Your policy schedule (in the portal) lists the exact amounts and how many visits per year your plan includes.

Because each policy pays on its own, one event can trigger several at once. A hospital stay pays the admission benefit, the daily benefit, your hospital indemnity lump sum, and your gap benefit, all separately.

In network, it works like regular insurance. Hand over your ID card, the provider bills ManhattanLife, and if the benefit is more than the bill, the difference comes to you.

Out of network, it pays exactly the same. You'll schedule as a self-pay patient (ask for the self-pay discount), pay, and upload the receipt for reimbursement. In-network is easier and cheaper, but you're never stuck.

What this plan is not. It isn't major medical and it isn't an ACA plan. There's no annual out-of-pocket maximum, so a long hospitalization or a major diagnosis can cost more than the benefits pay. That's exactly why the $2,500 rule exists — CareGuide routinely cuts hospital bills by 40–50% — and why we review your coverage together every year.

Finding a doctor.

Your plan uses the First Health network — one of the largest PPO networks in the country, with close to a million providers and about 6,000 hospitals. You will not have trouble finding someone near you.

Search the provider locator by zip code, or call First Health and they'll confirm a specific office or send you a directory. Interview two or three — everyone in the network has already agreed to accept this plan.

Say this at the front desk

"I have the ManhattanLife Affordable Choice plan. Do you participate in the First Health network?"Ask about First Health, not ManhattanLife. Front desks know the network name; they may not know the carrier.

Find an in-network doctor, urgent care, ER, and hospital before you need them, and you'll never think about the network again.

Prescriptions.

Your plan reimburses $50 per person per day you fill a prescription, up to the annual maximum on your schedule. Use any discount card you like, pay the discounted price at the counter, then photograph the receipt and upload it in the Easy Upload app. If your prescriptions cost less than $50 that day, you keep the difference.

Prices for the same drug vary wildly between pharmacies. Check before you fill — the cheapest pharmacy means the biggest reimbursement in your pocket.

Dental, vision & hearing.

If your package includes the DVH Select plan, it's first-day, first-dollar coverage: two cleanings, two exams, two sets of X-rays, and two fluoride treatments per person per year at no cost to you. Dentists are on the Careington network.
